Replacement Cards
PEX will issue replacement cards when cards are lost, stolen, or damaged by request.
Please contact your Administrator or PEX as soon as possible to report lost, stolen, or damaged cards, and to report fraudulent charges.
Cardholder should call 1-866-685-0898, available 24/7.
Administrator’s should call 1-866-685-1898, available 24/7.
If you need your replacement card delivered via Rushed, Expedited, or Priority delivery rather than First Class Mail, please contact your administrator. Rushed, Expedited, or Priority delivery of replacement cards will incur an additional fee.
